Taylor Swift premiered her cinematic video for ‘Patient Zero’ during the 2026 MTV Video Music Awards.
The dark production costars Dakota Johnson and Colin Farrell in a gothic story about a woman discovering the dangerous truth about her husband.
Swift appears as Farrell’s late former wife, whose ghost begins following Johnson and guiding her through the warning signs she once missed.
Matching clothes and mirrored scenes initially connect the two women before Swift’s true identity is revealed.
The video ultimately suggests that Farrell’s character was responsible for his first wife’s mysterious death and now stands to inherit her fortune.
Cara Delevingne also makes a surprise appearance as another woman encountered during an awards-show sequence.

The singer said the visual allowed her to explore gothic ghost stories, true crime, and psychological thrillers.
“The house was a metaphor for the coldness of a relationship that’s faded and atrophied.”

‘Patient Zero’ appears on ‘The Life of a Showgirl: The Encore,’ which arrived on September 25 with three additional songs.
Swift also received the VMAs’ inaugural Artist Director Honor and won Video of the Year for ‘The Fate of Ophelia’ and Best Direction for ‘Opalite.’
